THIS REGISTRATION IS FOR WOMEN'S PANHELLENIC RECRUITMENT!  
Please complete this if you are interested in joining the following organizations:  

Alpha Omicron Pi, Alpha Xi Delta, Delta Phi Epsilon, Gamma Phi Beta, Kappa Delta, Phi Mu, and Zeta Tau Alpha.

I understand that by clicking on the submit button on the bottom of this page, I am agreeing to the following as outlined by the Department of Fraternity & Sorority Life at Kennesaw State University.  Additionally, I agree to release the information contained herein to the relevant officials of the Panhellenic Councils' member chapters for recruitment purposes only.  This action, though, does not require me to join a Panhellenic organization, nor does it obligate any Panhellenic organization to offer me a bid to membership.

1) I understand that to be in accordance with the policies and procedures of the Department of Fraternity and Sorority Life and the College Panhellenic Council, all individuals interested in membership into a fraternity must have a minimum GPA of a 2.5. 

This number is based on an individual’s most recent cumulative GPA, whether that is their high school, transfer, or Kennesaw State GPA.  It is important to note that the Department bases our GPA calculation in conjunction with Kennesaw State University, meaning for high school and transfer GPAs that only core, non-elective classes are counted.  For the Kennesaw GPA, the department utilizes the adjusted cumulative GPA for membership purposes.

Additionally, while the above listed GPA is the minimum requirements to receive an invitation of membership into an organization, having this GPA does not guarantee membership.  For many of our organizations, a new member must have a GPA that is at least equivalent to their organization’s average GPA can be found at http://greeklife.kennesaw.edu/about-us/academics.php.  Due to this number constantly changing, other than publishing the minimum GPA, the Department is unable to confirm the actual GPA needed to receive an invitation for membership into an individual organization.

If you register and pay for recruitment and do not have the minimum required 2.5 GPA, you will not be allowed to participate in recruitment and will not receive a refund.

2) I agree to pay the nonrefundable recruitment fee of $100.00.  This is a nonrefundable fee, regardless of circumstance.  

3) I have read and agree to all the below agreements and disclosures.

My submission implies my compliance, initials, and signature on the below document. 
If I join an organization, this permission will be upheld for as long as I am an active member.

FERPA Disclosure

The Family Education Rights and Privacy Act of 1974 (FERPA) prohibits the release of personally identifiable information from students’ education records without their prior written authorization. Exceptions to this policy are limited to: (a) Release of such information to a specific list of officials with a legitimate educational interest in the record; (b) The release of such information in response to a court order, health or safety emergency, or approved research project; and (c) The release of public directory information, which has not been previously restricted by the student. An updated list of the categories of information that Kennesaw State University considers “directory information” is available online at:https://ferpa.kennesaw.edu/

Consent to Release of Information

By signing up for recruitment, I hereby give the Department of Fraternity and Sorority Life at Kennesaw State University (FSL) permission to release any personally identifiable information I provide to FSL and any organization within FSL during recruitment and/or membership activities, including, without limitation, my adjusted cumulative grade point average (GPA), contact information (including my phone number, email address, and physical address), payment information (such as my credit or debit card number), photos, videos, audio recordings, usernames and passwords, activities, contact information, student ID, preferred pronouns, survey responses, and community and chapter interests (collectively, “Data”), to the Disclosure Parties listed above and as otherwise allowed under FERPA. I understand that, with this release, my Data can be used by FSL for any and all university and departmental purposes including, without limitation, for university affiliation authorization, chapter scholarship statistics, educational programming, awards recognition, and verification of minimum academic standards and requirements.

 

Duty of Care

As a potential member or member to any organization related to the Department of Fraternity and Sorority Life, I understand that I must comply with Kennesaw State University’s Student Codes of Conduct. I will report any complaints or issues of misconduct to the Department and/or any other appropriate channels in the university.

Hazing Policy Notification

Kennesaw State University students and those who are engaged in activities with KSU students, including fraternities and sororities, are subject to federal and state laws, as well as the policies of the Board of Regents of the University System of Georgia and KSU, which prohibit hazing.   Under KSU’s Hazing Policy, hazing is defined as any intentional, negligent or reckless action, activity or situation, occurring on or off campus, that endangers or is likely to endanger the physical health of an individual or coerces the individual through the use of social or physical pressure to consume any food, liquid, alcohol, drug, or other substance which subjects the student to a likely risk of vomiting, intoxication, or unconsciousness or causes an individual pain, embarrassment, ridicule or harassment as a condition or precondition of gaining acceptance, membership, office or other status in a school organization or student group, whether or not such group is formally recognized by the University and regardless of the individual’s willingness to participate, and is strictly prohibited. The expressed or implied willingness of the victim(s) is not a defense. Any suspected violation of this policy should be reported to the Department of Fraternity and Sorority Life and/or the Department of Student Conduct and Academic Integrity and will be handled in accordance with the KSU Codes of Conduct, the Hazing Policy, and applicable law. I acknowledge this notification and agree to abide by KSU’s Hazing Policy

Photo/ Video/ Audio/ Media Release

I hereby give permission to the Department of Fraternity and Sorority Life at Kennesaw State University to publish any photos, videos, or audio recordings taken of me to any social media outlets, publications, or websites maintained by the Department or Kennesaw State University.  

I understand that this Consent to Disclosure and Acknowledgement must be submitted for me to have access to FSL services and information for recruitment and membership activities, authorizes my Data to be released during the following times: when I am 1) intending to be an enrolled student (for purposes of recruitment periods before the start of one's first semester); 2) an enrolled student; and/or 3) after I leave Kennesaw State University, and it will remain in place for one year unless I submit a written revocation of this Consent to Disclosure and Authorization to the Department of Fraternity and Sorority Life.
